If you signed up online as a new patient, paperwork will be sent to you ASAP!
If you’ve called to book your first appointment, please supply us with your email address and you will receive paperwork directly from us. With your online account, you will have the option to opt IN or OUT of reminders, etc.
Not able to do paperwork at home? No worries. We will have you set up with a tablet upon your arrival.


We are currently IN NETWORK with Medicare and most United Health Care plans.

We are not accepting new medicaid patients at this time.

It is our Office Policy to go through Auto Insurance for auto related injuries. Please click the links for more information.

*If your injury is due to a current auto accident, worker's comp, or personal injury case, please contact our office as we will require claim information prior to your visit and additional paperwork will be provided for you at the time of service.


Being in-network with some insurance companies is a courtesy we do for our patients. However, by doing so, insurance companies dictate how we can provide care. Some plans will require us to get approval for the care you need prior to you being treated.
We do prefer to treat on the first visit. The doctor may also do some soft tissue treatment such as Active Release Technique, Low Level Laser Therapy or kinesiotaping. Please be advised that your insurance plan may not reimburse for certain treatments other than spinal adjustments.

 

New Patients

We ask that you please arrive 15 minutes EARLY to your first appointment to make sure you have adequate time to complete any remaining paperwork. Please do bring your insurance cards if you plan to use them as well as any medication lists or other information you would like our doctor to have. Payment is due at the time of service.

Fees & Payments

Payment in full is due at the time services are rendered unless we are submitting claims to your insurance company. Copays and deductibles are due at time of service or your appointment may be rescheduled. Billing your insurance company does not ensure payment from them and in cases of a partial payment made by your insurance company, you are responsible for the remaining balance. You are responsible to inform us of any changes to your insurance prior to your visit with the doctor. Failure to do so may result in an unpaid claim that will become the patient's responsibility.

The doctor is certified in many other soft tissue treatments and modalities that may not be accepted by your insurance company. If ancillary treatments are desired by the patient or deemed necessary by the doctor, they may be rendered on a CASH basis and payment is due at the time of service. Many of these treatments expedite the healing process.

Appointments & No Show Policy

We respect our patient's time and strive to be on-schedule and ask that our patients to do the same. We ask that you arrive early to your scheduled appointment time. If you are more than 5 minutes late, the Doctor may take the next scheduled patient and you may have to reschedule. If you are experiencing a delay, please contact us and inform us of your estimated time of arrival. 

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the Doctors' day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be FULLY charged for their appointment.

Three No Shows may lead to your dismissal from our clinic. We do understand that sometimes circumstances are beyond your control. Please call and inform us.  


We urge you to call for appointments. Walk-Ins may be accomidated but there is no guarantee that you may be seen at that time.
